    Forms must be returned within 24 hours of booking to confirm your appointment.

Scheduling an Appointment

If you are ready to schedule an appointment with Dr. Salzer, please click “book an appointment online” to be taken to an online scheduler where you may choose an appointment time that works for you.

After booking your appointment please return here to complete the required intake & payment form and 4 consent forms. (estimated time 10 minutes)

  • An Intake & Payment Form with your basic demographic information, method of payment, and some policies of the practice to be initialed
  • HIPAA privacy form
  • Consent form for teletherapy
  • Cancellation policy
  • Prescription refills form

These forms must be completed within 24 hours to confirm your appointment and hold your spot.

Once those forms are completed, you will be emailed instructions on how to log on via a HIPAA compliant portal especially made for doctors and patients for your first session.


Payment for self-pay clients is due at the time of treatment and is billed to a credit card or health spending card kept on file for that purpose. The cost per 45 minute session is $500.

At the end of each month, upon request, we can provide you with a superbill to submit to your insurance if you have an out-of-network benefit for mental health. This will enable them to reimburse you directly based on the specifics of your health insurance plan.
